NSW SCHOOL VACCINATION PROGRAM 2018
NSW Ministry of Health offers all school students free vaccinations recommended by the National Health and Medical Research Council (NHMRC). These vaccines are important and protect students through to adulthood.
- Visit 2: Meningococcal for Years 10 and 11 – 14th August
Consent Forms
Please read the information provided with the consent form to understand facts about the vaccines that will be offered and the diseases that these vaccines protect against before you sign the forms.
Please note that all consent forms will need parent/gardians signature for each vaccine requested. The nurses will not be able to vaccinate your child if this is not done.
A Record of Vaccination will be provided to each student vaccinated at each clinic either as a physical card or a text message sent to the mobile number recorded on the consent form (within 1 business day). Parents/guardians should ensure that this record is kept for future reference and should not assume that their child has been vaccinated if they do not receive this Record of Vaccination. Keep this vaccination records in a safe place, as they will be required for future work or travel.
